How can employees access their pay stubs and tax forms?

Employees can access pay stubs and annual tax forms through the Employee Self Service (ESS) portal. Instructions for creating an ESS profile are available on the Payroll Services page.

What billing services are provided by EAK?

EAK handles all necessary billing functions for CDS clients, including verifying monthly insurance eligibility and submitting billing to the client’s insurance company.

What is the mission of EAK Consumer Directed Services?

The mission is to partner with and empower clients to successfully self-direct their care by providing incomparable customer service as their chosen Financial Management Services Agency (FMSA) and caring for clients and their employees.

The office is open Monday through Friday from 8:00 am to 5:00 pm, with a lunch closure from 12:00 pm to 1:00 pm.
